After taking the pretest you can use this document as a quick reference. It does not cover all topics in the VAERS Unaware education.

Review this resource before or during patient education on VAERS. It mirrors the Patient VAERS Report Information Sheet.

Complete and give this resource to your patients after you have made a VAERS report. A copy can also be used as documentation for the patient's permanent record.
